How do you extend a counter top over a built in dishwasher?
Why don't the top of the wall cabinets line up with the top of the pantry cabinet in 3d view? Wall cabinets appear mounted higher.
Does a refrigerator cabinet exisit?

I am not sure that all these solutions can be done in Instant Kitchen and Bathroom but I suspect they can.
1) Counter Top: If you add a new cabinet in the catalogue (or adjust one in the model) one of the options in Properties /Type is Counter Top. You can create just a counter top. You will need to elevate it and set it to the same dimensions etc as your exisiting counter tops of course. Having said that I would not do it that way as it can be a challenge getting the settings the same. All I would do is simply turn off Collision (in Status bar bottom right) and then move the dishwasher into an existing cabinet. If you leave the front of the diswasher protruding slightly in front of the cabinet you will have a built-in diswasher (which answers question 4 also). You can use the Nudge (arrow keys on the keyboard) to nudge it in clowly to align exactly. The Nudge can be set down to 1mm if you want, in Settings / Program Settings / Drawing Aids (assuming this is in Instant K & B).
2) Cabinet alignment: Cabinets can be any height so these will not default to the same - You have two options, make the pantry higher or Elevate the wall cabinets (right click Elevate + or - to go up or down)
3) Refrigerator Cabinet: Usually refigerators here (Australia) are not in cabinets to allow air circulation but you can make any cabinet any size, with or without doors, so just adjust one according to your needs, in the Catalogue or in the model.
4) Dishwasher Cabinet: The answer here is a combination of 1 and 3.
